近日英國媒體廣泛關注 Jessica Brady 的悲劇,一位27 歲年輕女子因 GP 診斷延誤,最終自費 MRI 才發現癌症已全身擴散。呢單新聞讓我聯想起兩年前在英國倫敦診治過的一位 27 歲 BNO 男性患者。 當時他因兩腳肢麻痺求診,症狀由腰部放射至兩腳腳踭。從初步物理檢查來看,表現極像坐骨神經痛(Sciatica)。然而,在臨床診斷中,我們必須時刻警惕那些隱藏在常見症狀背後的「紅旗徵兆」(Red Flags)。 臨床上的兩難:病人的堅持與專業的直覺 初診時他的神經受壓的範圍廣且症狀突發,我即時強烈建議他安排磁力共振(MRI) 影像檢查。然而他當時正準備月底與在香港居位的媽媽一齊前往美國參加畢業旅行。對一位年輕人而言那是人生中極其重要的里程碑,他選擇了先觀察。 出發前數天,他接受第二次治療。儘管施予了針對性的手法治療,但症狀緩解程度遠低於預期。這種「治療反應不佳」(Poor response to conservative treatment)在臨床上是極其重要的診斷指標。我再次嚴肅要求他回英後必須立即照 MRI,絕不可再拖延。
